A new independent review of the security arrangements surrounding the fatal shooting of conservative activist Charlie Kirk at Utah Valley University has identified significant gaps in event planning, communication and coordination.

Kirk was shot and killed on September 10, 2025, while speaking at an outdoor event at Utah Valley University in Orem, Utah. The new 158-page review was commissioned by the university and examined its planning, preparedness, response and recovery before and after the shooting.

According to the review, university officials had raised concerns about holding the event outdoors and suggested alternative locations that would have been easier to secure. Turning Point USA (TPUSA), the organization that hosted Kirk’s appearance, insisted that the event remain in the outdoor Fountain Courtyard. The report said that decision created additional security challenges.

The review also found that UVU did not receive reliable real-time information about expected attendance because TPUSA preferred to manage its own ticketing. University officials had initially been given an estimate of about 600 attendees, but ticket information was not subsequently available to police for accurate crowd planning.

Another issue involved a security meeting that never took place. UVU Police Chief Jeff Long contacted a member of Kirk’s private security team and proposed meeting at the elevated Hall of Flags walkway above the event area. According to the report, the meeting was supposed to take place at 11 a.m. on the day of the event, but it did not occur.

The report said UVU should have maintained greater control over important security decisions, including venue selection, ticketing, access control and other conditions affecting campus safety. It also identified inconsistent implementation of existing university security policies and insufficient documentation of the event’s security planning.

At the same time, the independent review did not conclude that UVU employees acted in bad faith or with willful and wanton disregard for safety. The university said the review also recognized its existing safety resources and the response of public safety partners after the shooting.

Kirk’s family has disputed the university’s handling of security and is planning legal action. Family attorney Brett Parkinson said the report supports the family’s allegations that the university failed to adequately protect Kirk and other attendees. UVU President Jon Anderson said the university would use the findings to strengthen campus safety and emergency preparedness.

The review made nearly 150 recommendations, including stronger event-security planning, clearer responsibilities and additional public-safety coordination. UVU has also created a new Chief Public Safety Officer position and said other safety improvements are already underway.

Tyler Robinson has pleaded not guilty to aggravated murder in connection with Kirk’s killing. As of the September 25 report, no trial date had been set.
